ไทย: แพรแถบย่อเหรียญกาชาดสมนาคุณ ชั้นที่ 1 ตามประกาศข้อบังคับสภากาชาดไทย แก้ไขเพิ่มเติม (ฉบับที่ 72) พุทธศักราช 2553 มีลักษณะดังนี้


ข้อ 90 เหรียญกาชาดสมนาคุณ มีลักษณะเป็นเหรียญโลหะกลมแบน เส้นผ่าศูนย์กลาง 27 มิลลิเมตร ด้านหน้ามีรูปกาชาดและรูปนางพยาบาลประคองทารกกับคนชรา ด้านหลังกลางเหรียญมีที่ว่างสําหรับจารึกนามผู้ได้รับเหรียญและพุทธศักราช ภายในวงขอบเหรียญเบื้องบนมีข้อความว่า “สภากาชาด” และภายในวงขอบเหรียญเบื้องล่างมีข้อความว่า “สมนาคุณ” ด้านหน้ามีห่วงเป็นรูปกระหนก สําหรับบุรุษใช้ห้อยกับแพรแถบสีขาวกว้าง 30 มิลลิเมตร ด้านหน้าแพรแถบตรงกลางมีเครื่องหมายกาชาดลําดับชั้นที่ได้รับ สําหรับสตรีใช้ห้อยกับแพรแถบผูกเป็นรูปแมลงปอ
เหรียญกาชาดสมนาคุณ มีลําดับเป็น 3 ชั้น คือ
ชั้นที่ 1 ทองคําลงยาที่กาชาด ด้านหลังเหรียญให้จารึกนามผู้ได้รับและพุทธศักราชที่ได้รับ ด้านหน้าแพรแถบตรงกลางมีเครื่องหมายกาชาดขอบทอง
ชั้นที่ 2 เงินกาไหล่ทอง ด่านหน่าแพรแถบตรงกลางมีเครื่องหมายกาชาดขอบสีทองบน วงกลมพื้นสีขาวขอบสีทอง
ชั้นที่ 3 เงินรมดํา ด้านหน้าแพรแถบตรงกลางมีเครื่องหมายกาชาดขอบสีเงินบนวงกลมพื้นสีขาวขอบสีเงิน
การให้เหรียญทั้ง 3 ชั้น มีประกาศนียบัตรกํากับทุกชั้น
เหรียญกาชาดสมนาคุณทุกชั้นมีเหรียญจําลองและดุมเสื้อ
